Here are the steps for filling up the Canada immigration forms for skilled workers:

For the Skilled Worker Category you along with your spouse and dependent who are below 18 years of age have to fill up the form. The form you need to fill up is known as SCHEDULE A (IMM5669E)-BACKGROUND/DECLARATION. Different numbers are given to each section and you have to fill up the form one by one.

1. Full name: Here provision is made for filling the name of the principal applicant, spouse or the dependent. Check the boxes correctly, as the first box for name has to be filled up only by the Principal Applicant name. There are other boxes given for other applicants and hence you should fill accordingly. Once you have written your full name in the box then, you will have to write your full name again in your native language. For example, if you are an Indian with your native language Hindi then, you will have to fill the form in Hindi. If you are, filling the form online either you will have to get the name typed in native language from someone or have to take out the print and write it down by pen.
2.  There is a place for nickname or other name by which you are called in family or social circles. You have to write the other names provided in the nickname section. If you do not have any other name then, you can write not applicable.
3.  The next box is for writing your date of birth. Write it in proper sequence as given: year, month and then date.
4. After the birth date, you need to fill up the personal details about your father. Write down his name and date of birth in the given spaces.
