Abstract and Introduction

Abstract


Topics covered in this review include the relation of estimated glomerular filtration rate, proteinuria and outcome; sudden cardiac death; contrast-induced acute kidney injury (CI-AKI); imaging; clinical trials targeting cardiovascular disease in chronic kidney disease (CKD) patients; and treatment of ischemic heart disease and valvular disease. Several studies reinforce the importance of CKD in predicting mortality and make a case for redefining CKD to incorporate levels of proteinuria in the staging system. Another study provides support for using a combination of echocardiography and cardiac biomarkers for cardiac risk stratification in dialysis patients. Two studies reveal ongoing interest in and difficulty preventing CI-AKI. One study discusses chest ultrasound to detect pulmonary congestion and potentially guide ultrafiltration and volume removal in dialysis patients. Several clinical trials assess folic acid, statins and aspirin for prevention of future cardiovascular events in CKD patients. Finally, several studies compare the use of drug-eluting and bare metal stents in dialysis patients, discuss methods of surgical coronary artery revascularization in dialysis patients and describe testing allopurinol as a treatment for ischemic heart disease in non-renal patients.

Introduction


Summer is over in the Twin Cities of St. Paul and Minneapolis, and the honking Canada geese flying south in their iconic 'V' formations remind us that the Snow Furies will soon descend upon us. The lazy tenor of neighborhood Labor Day block parties has been supplanted by a furious cacophony of shrill Tea Party activity (thankfully muted in Minnesota), which threatens to upend serious discourse on legislative policy (including recent halting attempts at health care reform) as another US Election Day approaches. On a more positive note, also temporally coincident with the preparation of my fifth annual NDT column, two weeks after a Kidney Disease: Improving Global Outcomes 'Controversies Conference' (Cardiovascular Disease in Chronic Kidney Disease: What Is It and What Can We do About It?) was held in London, the American Heart Association Scientific Sessions in November featured a five-day 'International Congress on CardioRenal Cross Talk and Syndromes,' highlighting the importance of chronic kidney disease (CKD) in cardiology.

Some of the topics covered at the congress will be reprised in this fifth column. These include the relation of estimated glomerular filtration rate (eGFR), proteinuria and outcome; sudden cardiac death; contrast-induced acute kidney injury (CI-AKI); imaging; clinical trials targeting cardiovascular disease in CKD patients; and treatment of ischemic heart disease and valvular disease.
