    • 1). Visit the Social Security Supplemental Security Income website at ssa.gov and complete the "Adult Disability Report" online. This report requires information including your name, social security number and information about the nature and history of your disability.

    • 2). Visit your local Social Security office and complete an application for benefits. You must call your local Social Security office toll free at 800-772-1213 to schedule an appointment to complete an application. Applications can be completed over the phone or in person. You must have your social security card and your medical history, work history and educational history when you visit your office to complete an application.

    • 3). Wait for a decision on your case. If your benefits are approved, you will be sent an award letter outlining your benefits. It will also indicate when you can expect your first payment. If your benefits are denied, you will receive a letter explaining why your benefits were denied and your options for appealing the decision.
