    • 1). Visit the North Carolina Vital Records website. Click on the "Order a Certificate" link, on the left-side of the page.

    • 2). Select either an English or Spanish application form under the "Death, Divorce and Marriage Certificate Application Form" header. The application form will be presented in PDF format, so you will need Adobe Acrobat Reader installed on your computer to view the file.

    • 3). Print the form. If you do not have access to a printer, visit a local library. For a small fee you will be able to print the application form.

    • 4). Complete the form with valid information. You will need to fill in information under the "Divorce Certificate" portion of the form. The "Required for All Certificates Requested" section of the form will also need to be filled in. Do not include any information under the "For Office Use Only" area.

    • 5). The fee is $24 as of 2010, payable by check or money order. Make it payable to "NC Vital Records."

    • 6). Mail the completed form and payment to the following address:

      North Carolina Vital Records

      1903 Mail Service Center
