The 'secret' Ingredient In Perio Disease

The causes of halitosis are numerous, but by far the most common cause is the presence of volatile sulfur compounds (VSC). VSCs are gases that result when anaerobic bacteria metabolize sloughed epithelial cells and decaying food particles. The gases consist primarily of hydrogen sulfide, methyl mercaptan, dimethyl sulfide and dimethyl disulfide. For example, hydrogen sulfide is the gas that creates the smell of rotten eggs and methyl mercaptan is the smell we experience in barnyards. No wonder halitosis can be embarrassing and offensive.

Research has also show a strong link between the presence of VSC and the occurrence of periodontal disease. Even in a low number, VSCs have been shown to have a negative effect on the oral mucosa's ability to resist bacterial invasion. In fact, methyl mercaptan is produced primarily by periodontal pathogens[1].

Even more disturbing is the news that the permeability of mucosa is increased by up to 75% following exposure to hydrogen sulfide and 103% with methyl mercaptan due to the VSC concentration[2].

With this evidence, it becomes very clear that eliminating VSC is not just about creating fresh breath. It is also a critical component in the fight against periodontal disease.

In the March 2010 issue of Hygienetown magazine, Trisha O'Hehir wrote about gingival bleeding and the fact that when the perio bacteria penetrate the junctional epithelium a series of events cause the bacteria to be able to flow into the blood stream through the increased capillaries caused by the inflammation.

In a research study review in the March Perio Reports, she summarizes the study "Oral Malodorous Compound Inhibits Osteoblast Proliferation" or "Bad breath might inhibit bone regeneration". In the review she states that VSCs initiate the periodontal pathogenesis process by enhancing permeability of the junctional epithelium and that VSCs inhibit osteoblast proliferation.

Ok- so why do you care? If you're not eliminating the VSCs you're simply not going to get the best results possible with your non-surgical and surgical perio therapy.
