    • 1). Download Form 9465 from the IRS website. If your total tax bill is more than $25,000, including penalties and interest, download Form 433F as well.

    • 2). Gather information about your financial status. Ideally, all of this information should be documented in case of an IRS audit. You will need to know your annual income, the total value of your property and the amount of your debts.

    • 3). Complete Form 9465 and, if you owe more than $25,000, Form 433F. Don't forget to include penalties and interest when you calculate your total tax due. Form 9465 requires you to provide a bank account number and authorize the IRS to make automatic withdrawals.

    • 4). Collect Form 9465, Form 433F, your tax bill and copies of your financial documentation, and mail them to the IRS at the address printed on Form 9465. Include Form 1040 if it is due and you haven't filed it yet.

    • 5). Modify your payment plan if the IRS requests that you do so. The IRS may ask you to pay higher or lower installments than you requested, depending on its assessment of your financial status.
