- 1). Collect the personal information of the deceased. This includes Social Security number, date of birth, date of death, any aliases and addresses of residence.
- 2). Gather the proof of death. Acceptable forms of proof of death include obituaries, death certificates, recognized sources that can be documented, written media, "Who's Who in America," an FBI file that indicates a person is dead, a date of birth going back 110 years or more, or a Social Security Death Index page.
- 3). Access the eFOIA form or acquire a hard copy of the FOIA form to mail in.
- 4). Fill either version of the form out with the personal information you've gathered.
- 5). State on the form how much you're willing to pay for printing fees if any are assessed; see details in the FBI's FOIA Reference Guide. Generally, the FBI provides the first hundred pages free of charge, with each additional page costing ten cents each. However, it sometimes waives the fee if there aren't many additional pages beyond the first hundred.
- 6). Submit your form via your chosen method.
