- 1). Visit the Internal Revenue Service website and download a new W-4 form.
- 2). Read the instructions that come with the form. Fill in the empty fields of the form, on your computer or by hand, to change your name (if you have been married or divorced) or address. Use the provided worksheets to ensure your withholding information is accurate.
- 3). Check over the form to ensure you filled it in correctly. Sign and date the form when you know it is accurate.
- 4). Give the new W-4 form to your employer. Your employer must file the form with the IRS. The new W-4 form must become effective by the beginning of the first pay period that ends 30 days or more after you submit the W-4 to your employer.
