- 1). Visit Social Security Online and read the Benefit Publications Section article "Tips for Completing the Application for Extra Help with Medicare Prescription Drug Plan Costs (Publication No 05-10122)."
- 2). Read SSA Publication No. 05-10128 to discover what you need to complete the application. Complete the worksheet within this publication to help you decide if you are eligible and to assist you in completing the actual application for extra help.
- 3). Complete an online application through the Social Security website, or through BenefitsCheckUp.org. If you prefer to apply by telephone, call Social Security at 1-800-772-1213 (TTY 1-800-325-0778).
- 4). Choose a Medicare prescription drug plan. If you need information about available plans or how to enroll in a plan, call 1-800-MEDICARE (TTY 1-877-486-2048) or visit Medicare.gov. If you do not select a plan, the Centers for Medicare and Medicaid Services (CMS) will do it for you.
