    • 1). Fill out a form from LexisNexis. This company is one of the most comprehensive data brokers operating in the U.S. To find the application, go to personalreports.lexisnexis.com. Select "Access Your Personal Information." Then click "Full File Disclosure Request Form."

    • 2). Copy identification documents. LexisNexis requires two forms of identification, one of which must be a government-issued identification card, the other a proof of mailing address. You must be able to provide a copy of your driver's license, Social Security card, military ID, passport, or state-issued ID.

    • 3). Find a proof of mailing address. These must have your name and mailing address on them. A valid proof of mailing address includes utility bills, an insurance declaration page, credit card bills, cell phone bills or your most current property tax bill. All bills must be dated within two months of your request.

    • 4). Mail your completed application. The full file disclosure is only available through the mail. Include a copy of your identification from steps 2 and 3. Make sure you have filled out the entire application. The mailing address is:
