Novel Therapeutic Targets in the Management of Atrial Fibrillation
JTV-519/K201 is a multichannel blocker which inhibits the activity of IKr and IKACh and also stabilizes RyR2. In a pre-clinical study by Nakaya et al., JTV-519 was observed to decrease the prolongation in APD induced experimentally by carbachol. The addition of JTV-519 to the guinea-pig atrial cells in this study also increased the carbachol-induced shortening in ERP. Both these mechanisms contribute to the efficacy of JTV-519 in mitigating the perpetuation of AF. The antiarrhythmic potential of JTV-519 was further demonstrated on the activity of pulmonary veins in isolated cardiomyocytes by Chen et al.. This was mediated by its inhibition of ICaL and NCX currents in the pulmonary veins ( Table 5 ).
