How to Apply for Disability in the State of Alabama
- 1). Make an initial determination that you qualify for disability under the federal guidelines. In general, you cannot be earning a substantial amount of income and you must have a medical or mental condition that prevents you from working for at least one year. The Social Security Administration website includes a short set of questions to help you determine if you might qualify (see Resources).
- 2). Decide whether you are going to apply online or at a local Social Security Administration office. If you choose to apply in person you may call first for an appointment or apply as a walk-in. The Social Security Administration website has a list of all offices in Alabama and their contact numbers (see Resources).
- 3). Visit one of the local Social Security Administration offices to complete the application or complete it online (see Resources). When you apply you will need your most recent W-2 form, Social Security numbers for your spouse and children, military discharge paperwork, and bank account information if you are requesting direct deposit. You will also need to provide information regarding your medical or mental condition, including treatment information and medical provider contact information.
- 4). Complete the Adult Disability Report that provides information regarding your condition and work history. You will also need to sign an authorization to disclose information to the Social Security Administration and mail it in if you complete the application online.
