• 1). Download the death certificate application, linked in the References section. You will need Adobe Acrobat reader to download the file.
  
  
• 2). Print the death certificate application.
  
  
• 3). Fill out the application with accurate information.
  
  
• 4). Make a copy of a valid government-issued photo identification.
  
  
• 5). Prepare a check or money order for $15 and make it payable to the Vital Records Service.
  
  
• 6). Prepare and enclose a self-addressed, stamped envelope. This is required for all mail requests.
  
  
• 7). Mail the completed form, copy of identification, payment and self-addressed, stamped envelope to the following address:

• 1). Download the death certificate application from the state of Oklahoma website. You will need Adobe Acrobat reader to download the file. Fill out the application before you visit a local office.
  
  
• 2). Prepare payment and identification for your death certificate request. Make out a check or money order for $15 payable to Vital Records Service. Ready your identification for your visit to the Vital Records Service office. You will need a valid government-issued identification card.
  
  
• 3). Visit the Vital Records Service office during the hours of 8:30 a.m. and 4 p.m. CST, Monday through Friday. The locations of these offices are as follows:
