How Do I Get a Death Certificate in Oklahoma?
- 1). Download the death certificate application, linked in the References section. You will need Adobe Acrobat reader to download the file.
- 2). Print the death certificate application.
- 3). Fill out the application with accurate information.
- 4). Make a copy of a valid government-issued photo identification.
- 5). Prepare a check or money order for $15 and make it payable to the Vital Records Service.
- 6). Prepare and enclose a self-addressed, stamped envelope. This is required for all mail requests.
- 7). Mail the completed form, copy of identification, payment and self-addressed, stamped envelope to the following address:

- 2). Prepare payment and identification for your death certificate request. Make out a check or money order for $15 payable to Vital Records Service. Ready your identification for your visit to the Vital Records Service office. You will need a valid government-issued identification card.
