How to Obtain a Copy of a Birth Certificate in AR

    • 1). Determine if you are eligible to apply for an Arkansas birth certificate. Eligible parties include people named on the birth certificate (the child and the parents), immediate family members, legal representatives (such as attorneys, legal guardians or estate executors), and academic research groups.

    • 2). Decide which type of birth certificate you wish to apply for--a certified copy or a wallet card. A certified copy includes a raised seal and is useful for all legal purposes. A wallet card does not have a raised seal and is useful primarily as a memento.

    • 3). Download a birth certificate application form the website maintained by the Arkansas Department of Health. You will be expected to supply the name of the person whose birth was recorded, the date of birth, the city and county of birth, the names of the parents (including the mother's maiden name), your relationship to the person whose birth was recorded, and your reason for requesting a birth certificate.

    • 4). Prepare a check or money order in the amount of $12 (plus $10 for each additional copy) made out to the Arkansas Department of Health.

    • 5). Mail the application together with your check or money order to the Arkansas Department of Health, Vital Records Section-Slot 44, 4815 West Markham Street, Little Rock, AR 72205. Allow four to six weeks for delivery. If you appear personally at this office before 4 p.m. on a weekday, the birth certificate will be issued the same day.
