How to File a Labor Law Grievance in Kentucky
- 1). Gather all relevant information to make a case against your employer. If the complaint regards underpayment of wages, bank statements and past pay stubs would be helpful. For overtime, commissions or other items, include any records, documents or logs that corroborate your hours, sales or disputed items.
- 2). Prepare the complaint form, found at the Kentucky Department of Labor website. Print the form and fill out all the necessary information. Include your name, address, phone number and Social Security number, as well as the amount of your wage dispute and a thorough description of the complaint. Fill out as much of the form as possible about your employer.
- 3). Mail the completed form to the address listed at the top. The Division of Employment Standards will investigate your claim and be in contact with you after it receives the form for possible further action. If your claim has validity, the state will likely be able to directly enforce the payment of your wages.
