How to Provide the Document Forms for a Motion to Terminate Guardianship

    • 1). Go to the office of the clerk for the civil court in the county where the guardianship is taking place -- or the county in which you live. The division of the civil court oversees family court and guardianship matters.

    • 2). Provide a minimum of two forms of identification to the clerk -- a driver's license and a social security card, for example. Tell the clerk the details of your situation -- whether you are a current guardian or someone other than the guardian wishing to end the guardianship. Give the clerk the guardianship case number. Ask the clerk for the correct Petition to Terminate Guardianship as pertains to your case. Request a Financial Accounting form -- if necessary -- to disclose financial matters associated with the minor's personal estate.

    • 3). Ask the clerk if forms must be typed. Request electronic versions of the petitions if form answers must be typed. Certain courts -- the Civil Court of Clark County, Nev., for instance -- will not accept petitions completed via pen or pencil.

    • 4). Fill out each petition in its entirety. Detail the reasons you wish to terminate the guardianship. Sign and notarize each petition.

    • 5). Make three copies of every document -- including financial and/or medical documentation you may use to support your motion -- and return them to the clerk as one package. Give two copies of documents to the clerk -- one for court records and one (if necessary) to the defendant. Keep the third copy for your personal records. Inquire with the clerk about a court hearing date to take the matter before a judge.
