How to File a Name Change Petition in Tennessee
- 1). Log on to the website of the chancery court for your county in Tennessee. Search the website for the name change petition form. If your county does not have a website, then go in person to the court and request the name change petition form from the clerk of the court. You do not have to be a resident of that county to file for a name change there
- 2). Fill out the name change form truthfully and completely. State your current name, county of residence, any aliases, as well as the reason for the name change.
- 3). Sign and have the form notarized by a notary public. Submit the form to the clerk of the court for approval. Once approved, you may then change your name with the Department of Motor Vehicles.
- 4). Go to the nearest motor vehicle department office. Bring your driver's license or state identification, and the official name change form approved by the clerk. Fill out the duplicate license/identification form. Pay a fee for the duplicate license/identification.
