How to Obtain an Acknowledgment of Paternity Form in New York
- 1). Download Form LDS-4418 (see Resources) if the child is born outside New York City or Form LDS-4418 NYC (see Resources) if the child is born in New York City. The Division of Child Support Enforcement provides printable PDF versions of the voluntary acknowledgement of paternity form, which may be filled out and filed with the Division of Child Support Enforcement. You may also obtain either form from the hospital where the child is born or a child support office.
- 2). Provide information about the child. Write the child's full name as it appears on the birth certificate. Indicate the name and address of the hospital where the child was born, the date of birth and sex of the child.
- 3). Fill out the "Acknowledgment of paternity by father" section. Provide the full name, address and Social Security number for the father. The father must sign in the presence of two witnesses that are not related to either the mother or father.
- 4). Fill out the "Acknowledgement of paternity by mother" section. Provide the full name, address and Social Security number for the mother. Indicate whether the mother is receiving child support services; provide the county and address of the social services district if the mother is receiving child support services. The mother must sign in the presence of two witnesses who are not related to either the mother or the father.
- 5). Provide a mailing address and phone number for the mother and father if filling out Form LDS-4418 NYC. On pages 3 and 4, provide the name, address and phone number for both the mother and father.
- 6). File Form LDS-4418 or Form LDS-4418 NYC. The form may be filed at either the hospital where the child is born, a Division of Child Support Enforcement office or county or borough registrar of birth records.
