How to Write on a Lock Tag

    • 1). Locate the breaker for the machine you wish to lockout. Turn off the breaker, removing all power from the machinery.

    • 2). Insert a padlock through the hole in the lock tag. Secure the tag and padlock to the breaker using the proper lockout procedure, as outlined by OSHA guidelines. If you are unsure about this procedure, contact your manager or supervisor for more information.

    • 3). Use a marker or pen to write directly onto the lock tag. Tags vary depending on the style, but most will contain a space for the date, name of person(s) who locked the machine, reason for locking and a space for the signature.

    • 4). Fill out the tag completely. If applicable, separate the bottom portion of the tag and present it to your supervisor or manager. Keep the key to the padlock with you until the machine is ready to be unlocked.

    • 5). Perform the necessary maintenance or repair to the machine once it is safely locked.
