How to Become a Home Care Provider in Oregon
- 1). Establish your business. Before you apply for an in-home care agency license, you must set up your basic business. This involves determining your business structure and getting a municipal or county business license.
- 2). Download, print and complete an Oregon in-home care agency license application. You can find this application on the DHS website (see Resources). On this application, you must include information on your business structure as well as information on what types of home care services you intend to provide. You must include a $1,500 fee and submit your application to the Oregon Health Care Regulation and Quality Improvement Division at P.O. Box 14260, Portland, OR, 97293-0260.
- 3). Undergo a business inspection. Once the DHS receives your application, it will contact you to arrange for an inspection of your business. A representative will check that you meet all of the requirements of an in-home care agency. The DHS will check that you have adequate administrative oversight in place, have done background checks and pre-screening for all employees, and that employees possess all required licenses or certifications relevant to their positions.
