How to Change Your Maiden Name Online on a Social Security Card
- 1). Fill out Form SS-5, the Application for Social Security Card. You can print the form from the Social Security Administration's website, visit a local office in your area or call the Social Security Administration at 1-800-772-1213 to have one mailed to you.
- 2). Show proof of legal name change. If you are newly married, a recent marriage document will suffice.
- 3). Show proof of identity. You will need a driver's license, state-issued identification card or U.S. passport for identification purposes. If you don't have one of these or can't get a replacement within 10 days, there are other forms of identification acceptable (see reference).
- 4). Bring or mail your application, marriage document and proof of identity to your local Social Security office.
- 5). Get a new card. Once your documents have been verified, you will be sent a corrected card in the mail. The card will have the same number but your new name will be listed.
