How to Retrieve a U.S. Birth Certificate

Visit the Center for Disease Control's "Where to Write for Vital Records" page. You'll find detailed contact information concerning the vital-statistics departments for states and territories within the United States. - 2). Find the state where the birth occurred and click the corresponding link.
- 3). Jot down the address to the appropriate department. Usually, the state's Vital Statistics Department or Department of Health handles birth certificates.
- 4). Visit the appropriate state website to print a birth certificate application, if available. If one isn't available, compose a letter to the department. Include your identity, your relationship to the person whose birth is listed on the certificate and your reason for requesting the certificate. Include a phone number and the best time to reach you.

Make a photocopy of your government-issued I.D. and include it, along with a self-addressed stamped envelope. Acceptable government-issued I.D.'s usually include a driver's license or state-issued I.D., a passport, passport card or military I.D. Please note that because birth certificates are private records protected by state law, a copy of your identification must be provided. - 6). Include the fees, payable in a cashier's check or money order. Be sure to contact the state directly to verify the appropriate amount.
- 7). Mail the packet to the appropriate address, usually the state's vital-statistics department.
