- 1). Fill out the Request for Driving Record application. You will need to know your Indiana driver's license number. Sign and date the form.
- 2). Write a check or money order for $4, payable to the Bureau of Motor Vehicles.
- 3). Mail the application and payment to:

100 North Senate Ave., Room N405
Indianapolis, IN 46204.
You can also take the application and payment to a local Bureau of Motor Vehicles office anywhere in the state.
