How to Do a Legal Name Change After a Divorce
- 1). In the divorce decree, make sure your change of name is stated in the divorce decree. Name changes can be as simple as, " xxxx xxxx will assume her maiden name of xxxx xxxx".
- 2). Make several copies of your divorce decree. You will need these as proof of a legal name change.
- 3). Request a new Social Security card or cards reflecting the social security name change. Call the Social Security Administration at (800) 772-1213 to obtain forms and instructions.

Update financial account information: bank accounts, certificates of deposit, personal checks, debit cards, credit cards, 401K accounts. - 6).
