104 4
    • 1). Find your birth entry number. This number is listed on the birth certificate, if you are requesting an additional copy, or on the certificate of registry. If you do not have access to or have lost this number, you can apply for the number by contacting the RGD, or Registrar General's Department.

    • 2). Gather the necessary information to fill out the application. This includes the full name of the person, the gender, the date of birth and the place of birth. You will also need the district of birth, the parish of birth, Mother's name, Father's name, and the place of registration.

    • 3). Fill out the form or application for the birth certificate. If in Jamaica, you can go to your local RGD office. If not in Jamaica or not able to travel to an office, you can apply online. You will need all of the gathered information on the individual listed on the birth certificate as well as the names of the birth parents.

    • 4). You will receive your birth certificate within ten to twelve weeks from the time you fill out the application. Do not file a request for another birth certificate during this time as it can slow down the process. Check over the information once you receive the birth certificate to make sure that is all is correct.
