How to Change a Name for Marriage in Florida
- 1). Make a list of everything you need to have changed after you have been married. Legally, you can start using your new last name the second you are legally married, so you should pre-plan and make sure you have noted everything that needs to be changed. List them in order of priority, with your Social Security card and driver's license as the top two.
- 2). Go to the Social Security office or have them send the correct form to get a new Social Security card with your new name. Fill out form S-5 by downloading it and printing it, picking it up or having it mailed to your home. Go to the office and show proof of the name change by providing the office with a certified copy of the marriage license for verification.
- 3). Take a certified copy of your marriage license to the Florida Driver's License office and show them the document to have your name changed on your license. Pay the fee, which can vary, and the office will issue you a new license with your new name. You will want to change it as soon as possible to avoid conflicts with the police or when using your license for identification purposes.
