Use the Online Form

• 1). Go to the Washington State Department of Licensing website. On the "Replace a Lost Title" page, click on the link to open the PDF file called Affidavit of Loss.
  
  
• 2). Fill out the Affidavit of Loss/Release of Interest form. You cannot save the form, so be sure to have all your information handy. Required information includes your license or registration number, the vehicle's VIN, year, make and model and the title number. All vehicle owners must provide their notarized signature.
  
  Instructions for new titles and title transfers are on the second page of the document. Read through for information relevant to your situation. This page is for reference only and does not need to be mailed to the DOL.
  
  
• 3). Print out the completed Affidavit of Loss/Release of Interest form.
  
  
• 4). There is a fee charged for the replacement title. Fees for replacement titles and registrations are listed on the DOL website. Fees vary, depending on where you live, the weight of your vehicle and the type of document requested. Use the table to determine which fees apply to you, and write a check or money order for that amount. Do not mail cash payments.
  
  Fees may be paid in cash only if the Affidavit of Loss is submitted in person to a DOL office.
  
  
• 5). Mail the completed form and a check or money order to any DOL office. Office locations, hours and contact information are listed on the DOL website, sorted by Washington state counties. Click on the picture of your county or select from the drop-down menu to view the office information.
  
  Completed forms and check or cash payments can be brought in person to any DOL office.
  
  
• 6). Your replacement title will arrive in the mail within eight to 10 weeks.
