How to Transfer a Patient

• 1). Complete the patient transfer paperwork. This should include discharge paperwork at the existing institution and registration paperwork at the receiving institution.
  
  
• 2). Register the patient at the receiving facility. Ensure that the new facility has the ability to match the required care of the patient that the transferring facility does.
  
  
• 3). Make 2 copies all forms and chart information and send one copy with signature confirmation via UPS, Fed Ex or the post office and one to remain with the patient through transport. Retain all original forms and chart information for the patient at the transferring institution.
  
  
• 4). Transport the patient, monitoring all vital signs, IV drips and medications as prescribed by the primary-care physician. Make sure discharge paperwork is signed at the transferring institution and admittance paperwork is signed at the receiving institution.
