- 1). Schedule a late doctor's appointment. Ask for the last appointment of the day, which gives you more time with the doctor. Take this opportunity to ask your doctor to complete your FMLA forms.
- 2). Fax your FMLA forms to your doctor's nurse. On the fax cover letter, include the fax number to the benefits office of your employer. In addition, specify a return date.
