How to Order a Birth Certificate in Quebec
- 1). Download the copy of an application form for birth certificate from the website of Directeur de l'état civil. See the first Reference for a link. Print the application form.
- 2). Determine the type of birth certificate you want to receive. There are two types of birth certificates. Short form birth certificates contain information on surname, given name(s), sex, date and place of birth, registration number, date of issue and document number. Long form birth certificates include all the information given on short form as well as names of father and mother of the person.
- 3). Choose the type of processing. Normal processing takes about 12 business days and the certificate is mailed to you using regular mail. Accelerated processing takes three days and the certificate is mailed to you through Xpresspost if you are resident of Canada and regular mail if you are living abroad.
- 4). Attach a photocopy of two identification documents. One document should be a valid photo ID while other should be a proof of your residence. Proof of residence can be a valid driver's license, municipal or school tax bill, government postal correspondence, recent invoice from energy, telephone or cable supplier, construction competency certificate, hospital card, recent pay stub, bank statement, home or car insurance certificate or a Canada Post change of address receipt.
- 5). Sign and date your application and mail it to following address.

- 2). Choose the birth certificate option, click the check box "I have read and accepted the terms of use" and click the "Begin" button.
- 3). Provide your clicSÉQUR ID for authentication purposes. ClicSÉQUR is authentication service of the Government of Québec. If you don't have a clicSÉQUR ID, you can obtain it by providing your social insurance number, Revenue Quebec access code and your notice of assessment number.
- 4). Fill in the form and pay the processing fees. You can only pay by your credit card if you are filing the application online. Press the "Submit" button to complete the submission process.
- 5). Use your confirmation number to track the status of your application. This confirmation number will be provided to you once you have paid the required fees and submitted your application to receive the birth certificate.
