- 1). Contact your local Ontario Disability Support Program office and schedule an appointment for a meeting. You can meet at the local office or another location more convenient for you. Give them notice if you need special help, such as an interpreter, large print forms and information, or braille.
- 2). Bring all of your documents to the initial meeting. The meeting will last approximately an hour and 20 minutes, and a case worker will go over your financial eligibility in detail. The criteria for acceptance depends on many things, such as size of family, income and assets, and where you live. You will be notified of your eligibility by the end of the first meeting. If you are denied, you can request a review.
- 3). Complete steps 4 to 6 within 90 days of the day you receive the Disability Determination Package.
- 4). Have your doctor fill out the Health Status Report and the Activities of Daily Living Index in the Disability Determination Package you will receive. These forms must be completed by a certified health professional, such as your family doctor, psychiatrist, ophthalmologist or optometrist.
- 5). Complete the Consent to Release of Medical Information form. This form must be completed by either you or your trustee.
- 6). Complete the optional self-report form which gives you a chance to fill in any missing information and help the Ministry in making a better determination. While you are not required to complete this form, it can help speed up the application process. The form can be completed by you or your trustee, your spouse, another family member or anyone else you trust.
- 7). Once all of the forms are complete, send them to:
