- 1). Download form 4506 from the IRS website.
- 2). Complete form 4506. Provide your identifying information, the tax form you need a copy of and the year or years you need copies for.
- 3). Calculate the cost of the copies by multiplying the number of copies you need by the cost of each copy. As of 2010, the cost for each copy is $57. For three copies, multiply $57 by 3 to get $171. Make checks payable to the United States Treasury.
- 4). Enclose payment with your form 4506 and mail it to the appropriate address based on where you filed your tax return. Find this information form 4506.
